HOW TO APPLY
The following documentation must be completed and emailed to Hesketh King Treatment Centre for the attention of the Admission Officer
- A comprehensive Psychosocial report (Admission form) completed by a registered social worker.
- Individual Development Plan (IDP) compiled by the external social worker.
- A medical certificate and dental form completed by a medical professional.
- A Psychiatric report by a qualified psychiatrist stating the psychological stability of the applicant if there has been a psychiatric diagnosis..
- Contract form (Completed by the family members). This can be completed on arrival.
- R1200 Patient contribution fee (non-refundable).
